Patient Information

Should you like to make an appointment for consultation or if you have general questions, please call us at (530) 758-2350.  Our office hours are Monday through Thursday, 8:00 A.M. to 5:00 P.M., and Friday, 8:00 A.M. to 4:00 P.M.  We will schedule your appointment as soon as possible.  If you are in pain, or in an emergency situation, we will make every attempt to see you that day. To cancel an existing appointment, please call at the courtesy of 48 hours in advance.

First Visit

Prior to your first appointment, we like to mail the following forms in the mail, as well as a card with the date and time of your appointment.

• Patient Get Acquainted Form
• Assignment of Benefits and Release of Information Form
• Medical History

Please bring these completed forms to your consultation, as well as any x-rays or referral slips to you by your referring doctor.  If the forms are not with you, please come 15 minutes early to your appointment to fill them out.

Before surgery

For patients having general anesthesia (IV or inhalation), you must not eat or drink anything, including water, 8 hours prior to surgery with the exceptions: 1) If you have medications to take, please take them, but with minimal water; 2) You may have clear fluids (any that you can see through) until four hours before surgery.  The patient must also have a ride from our office, as drowsiness is expected after the procedure. Someone should be available at home for 6-8 hours following anesthesia. It would be helpful if the patient wear a short-sleeve shirt to facilitate the use of IV access.

There are no preoperative instructions for patients with local anesthesia, however someone should be available by phone.
